We are excited to share some great news about our practice! Beginning October 5, 2026, Jackson Family Practice will be joining the Cleveland Clinic Primary Care Institute.

Our practice will have a new name: Cleveland Clinic Akron General Medical Outpatient Center – Jackson, but will remain at our current location.

 

To prepare our office for this exciting transition, we will be closed for renovations from October 1–19, 2026. We will reopen to the public on October 20, 2026. We look forward to welcoming you back to our newly updated space!

 

As we move forward, there are some changes we wanted to make you aware of.

After 36 years of practice, Dr. Crock will be retiring on September 4, 2026.

As of September 30, 2026, Jennifer Gonzalez, PA-C will be leaving our practice to pursue a great new opportunity. We wish them both the best in the next chapter of their lives and careers!

The APCM program will be discontinued and Jackson Lifestyle Solutions visits will transition to regular office visits starting October 1, 2026.

 

To continue your care with us, you may need to complete Cleveland Clinic's patient registration and insurance-verification process. This may take place when you schedule your appointment, before your visit, or when you arrive at the office.
