Below are answers  to the most commonly asked questions of patients who are new to our practice. We hope you will find this information useful.

  • What is Family Practice?

    Our receptionist can schedule you or your family member for a new patient appointment by phoning 330 966 1319, Monday – Friday from 8am to 4pm. We will need date of birth, address and insurance type.

  • What You Need to Know Before Making Your First Appointment

    Call the number on your insurance card to confirm that your insurance will allow you to see the providers at Jackson Family Practice. You want to confirm that JFP is “in network” for your specific insurance plan.

  • How to Make a New Patient Appointment?

    Our receptionist can schedule you or your family member for a new patient appointment by phoning 330.966.1319, Monday – Friday from 8am to 4pm. We will need date of birth, address and insurance type.

  • What To Bring to Your First Visit

    • Bring all medications, supplements or herbal medications you are currently taking
    • past surgical history, type and date
    • past medical conditions you have been treated for
    • know all allergy’s medical and environmental
    • know hospitalization history
    • know medical conditions your immediate family members (parents, grandparents, brother, sister) have been treated for
    • name of your pharmacy and the street location
  • What Happens at the First Visit?

    All new patient appointments are to establish as a patient at Jackson Family Practice and include a general physical exam. This is the first step in establishing a relationship with the office staff and providers at JFP. However, most insurances do not cover a visit with no diagnosis or symptoms, therefore we must have two reasons to see you as a patient. Reason one is to establish as a patient, reason two can be one other visit type (see visit types) in compliance with your insurance and our practice policy. An example would be a new patient to establish visit and a symptom visit to evaluate your knee pain.

  • Who Will I See at My First Visit?

    At this time all of our providers are taking new patients, you can schedule for the provider who meets your time schedule. However, be aware that the provider you see as a new patient does not have to be the provider you see for every visit. All of your providers will be familiar with you and your medical information so you can see the provider who best meets your schedule. At “follow up” visits you will see the provider who requests the visit.

  • What Time Should I Be at My New Patient Appointment?

    Please come at least 20 minutes before your scheduled visit time. You will spend that time completing your demographic and medical history privately on our intake computer (no computer skills needed).

  • Canceling a New Patient Appointment:

    We reserve a lengthy time frame in our schedule for our new patients. As a courtisy to your fellow patients and our providers it is imperative that new patient appointments be canceled at least 24 hours in advance. Failure to cancel and “no showing” a new patient appointment can mean that Jackson Family Practice will no longer be able to be your families physicians.

  • How Long Does an Appointment Take?

    Although our patient wait time is the lowest in the area, please allow at least 1.5 hours from check in to check out for your visit.

  • Transferring Records:

    It is not necessary to transfer your records from a previous provider before your new patient appointment, however we would like you to complete a transfer form at your visit.

    If you experience a life-threatening emergency, please dial 9-1-1 immediately. If you have an urgent health matter after hours please dial 330-966-1319. A live operator will answer your call and contact your physician should that be necessary.
